Community Support/Recovery Pathways (CS/RP)
RECOVERY SUPPORT
The Community Support/Recovery Pathways program offers mental health and substance use rehabilitation services to help individuals throughout the Greater New Haven area achieve and maintain the highest degree of independent functioning. Not only is the program tailored to address each participants needs and preferences, it is community-based so that the CS/RP team can become intimately familiar with each participant's surroundings, strengths, and challenges.
Key components:
- Individualized rehabilitative community support utilizing team approach
- Crisis intervention
- Individual & group mental wellness education
- Skill building for daily living, peer support & self-management
