Community Living Arrangements

DEVELOPMENTAL DISABILITY SERVICES

Group living for socialization and skill building

“Jeremy has had many challenges in his life. Continuum staff have been there to see him through. He has improved socially and has been able to learn and work. Jeremy has found a home that he loves and we greatly appreciate it.” - Joseph & Kathy C., parents

Continuum believes strongly in the power of community, and that all people deserve to be treated with dignity and respect. Further, it is our philosophy that all people with intellectual and developmental disabilities (such as Autism, traumatic brain injury, seizure disorders, and others) have the right to determine their own style of life as they live in the community.  Continuum’s Community Living Arrangement programs provide housing and supportive services tailored to each individual’s unique needs, interests, and capabilities.

The program offers individuals opportunities to live in personalized home environments in established residential neighborhoods throughout Connecticut, within close proximity to shopping, movies, and restaurants. Highly trained staff members provide assistance with personal needs, daily living skills, medical needs including medication administration, future dream building, personal relationship building, and self-advocacy. In-home intensive day programs can be developed based on need and ability.

Where possible, our residents are encouraged to become active members of their community through group activities, recreation, and volunteerism — for example, residents participate in Foodshare, distributing tons of food throughout the year to soup kitchens and pantries.

Our Program Offers:

  • Case management
  • 24-hour on-site staffing, including awake overnights
  • Person-centered planning
  • Daily skill building & self-advocacy coaching
  • Behavioral consultation (as needed)
  • Access to community, volunteer, & recreational activities
  • Collaboration with nursing agencies, psychiatric & medical providers, conservators, family & friends
