Continuum of Care, Inc.’s mission is to enable people who are challenged with mental illness, intellectual disability or other disability, and/or addiction, to rebuild a meaningful life and thrive in the community.
Community Living Arrangements
INTELLECTUAL DISABILITY PROGRAMS
Group living for socialization and skill building
Continuum's Community Living Arrangements offer individuals opportunities to live in personalized home environments in established residential neighborhoods in Hartford and New Haven Counties and are within close proximity to shopping, movies, and restaurants. Highly trained staff members provide assistance with personal needs, daily living skills, medical needs including medication administration, future dream building, personal relationship building, and self-advocacy. In-home intensive day programs can be developed based on need and ability.
Residents are encouraged to become active members of their community through group activities, recreation, and volunteerism — for example, residents participate in Foodshare, distributing tons of food throughout the year to soup kitchens and pantries.
