Retiring After 40+ Years of Rebuilding Lives, Patti Walker’s journey as the CEO
of Continuum of Care has been nothing short of extraordinary.  As she retires, we believe her legacy deserves
a celebration that echoes the transformative impact she has had on tens of thousands of lives.

When Patti assumed the role of CEO for Continuum of Care, she faced the challenge of managing only 1 house,
New Haven's first Halfway House, with just 13 clients in her charge. Today, under Patti's visionary leadership,

Continuum has evolved into one of Connecticut's largest and most comprehensive residential and support services
for individuals dealing with mental illness, ntellectual disabilities, and homelessness. Continuum now helps more than 2,400
people annually to rebuild their lives across 60 locations throughout the state, by providing housing and essential non-residential
support programs, including an outpatient clinic, in-home services, case management, crisis outreach and stabilization,
and recovering peer services. Patti's commitment to rebuilding lives has been the driving force behind this incredible transformation
.

Under Patti Walker’s 40+ years of leadership, Continuum has helped thousands of Connecticut’s most vulnerable citizens
diagnosed with mental illness and intellectual disabilities to rebuild their lives and live in the community as fully as possible.
